Intake Coordinator
Job Summary:
The Intake Coordinator ensures coordination of intake into the organization. The coordinator connects patients and caregivers with the appropriate services and provides essential resources for dealing with Alzheimer’s disease and related neurocognitive disorders.- Screens all first-time callers by gathering information necessary to determine the course of care.
- Assists with obtaining the medical information form.
- Connects patients and caregivers to the appropriate services within and outside of ACC.
- Responsible for maintaining patient and caregiver data and updating the electronic health record database as needed.
- Assist with other administrative projects relating to data entry, EHR, reporting, funding/case management, filing, etc.
- Travels to and serves as a patient aide at day centers to comply with patient-staff ratios under our specialized day center license regulations.
- Serves as a backup for front desk receptionist phone coverage as needed.
Education and Experience:
The Intake Coordinator is a professional with a high school diploma and/or equivalent to, two-three years of professional experience in administrative duties.Travel:
