PATIENT/CLIENT REPRESENTATIVE/COORDINATOR
JOB DESCRIPTION
POSITION SCOPE:
To ensure effective coordination of services working with the company’s personnel, and through liaison with other organizations and individuals also providing care to the patient/client. Also to participate in assessment and planning of services according to
Philadelphia Home Health Care Inc. philosophy, standards of home care, and Policy and Procedures manual, as necessary.
Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager, Administrator.
POSITION QUALIFICATIONS:
Graduate of a high school or with some years of high school education (no Diploma required)
Preferably experienced within home health setting (at least one year).
Evidence of knowledge of the services provided by the company. Ability to work under stress, and to take rapid decisions.
Verbal and written communication skills.
Good organizational skills.

Ability to deal effectively with stress and a great work load at times.
This Job Classification will not have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids.
PERFORMANCE ABILITIES AND STANDARDS:
Participates in coordination of services/care that the patient receives,
Understands (he principles o the service/Care provided, knows the qualifications required for the staff providing (he service, knows which stall possess these necessary qualifications for the type of service provided.
Knows the relationship between services provided by various staff members (when applicable).
Participates in warehouse activities directed to inventory issues.
Is responsible for confidentiality issues. Ensure HIPAA guidelines and procedures are maintained.
Understands the nature and type of the patient/client population serviced.
The patient/client representative will initiate communication with other organizations or individuals when there are significant changes in the patient’s/client’s overall care (if applicable) The patient/client representative will define the responsibilities of all organizations/individuals involved in patient’s/client’s care, arid will make sure that these responsibilities are clearly understood.
The patient/client representative will make sure that there are no duplications in services provided by interviewing the patient/patient’s family and/or case manager- ha duplication of service is found, immediate corrective action will be taken.
The patient/client representative will be responsible for the regular communication with the physician. The regular communication with the physician will be as frequent as the status, and the response to service of the patient/client warrants.
The patient/client representative will take reasonable steps to assure that care and/or services provided will be coordinated and integrated to the patient’s needs and available time.
The patient/client representative will take reasonable steps to assure that the policies and procedures that guide and support the provision of care and services will be implemented.
The patient/client representative will participate in education conferences, meetings, in-services and training for policy and procedures modifications, emergency response and preparedness plan, organization planning, quality assurance and company activities improvement, etc. The patient/client representative gives accurate information to clients, clients’ families, and other professionals involved in patient’s care.
Complies with all applicable Policies and Procedures, Federal and State rules, regulations, and laws in effect, with respect to home health care.
Promotes and participates in education activities for patient, Patient’s family, caregivers, and/or significant others (if necessary).
Participates in personal growth and development.
Documents all communications in the Communication Notes from the patients’ charts.
The patient/client representative will be a qualified person who understands the principles of care for home health services, has the education necessary according to the requirements of his/her job description, and applicable law and regulation, has knowledge and experience, and has the orientation to all the care and/or services lie or she will he coordinating, understands the staff responsibilities associated with each level of care and services provided, has knowledge of the patient population served, has knowledge, and experienced appropriate to defined responsibilities and according to the Agency’s policy, has access to qualified consultation for services outside his/her expertise (if necessary).
LINES OF AUTHORITY:
The Administrator appoints the patient/client representative.
The authority conferred by the Administrator is commensurate with his/her responsibility for the overall operation 0f the Agency demonstrated by the job description, and by the organizational charts.
The patient/client representative appointed by the Administrator has the necessary authority to implement his/her responsibilities for the operation of the Agency’s Patient Service Department.
EVALUATION OF PERFORMANCE
The performance of the patient/client representative shall be monitored regularly through a procedure established by the Administrator.
The mechanism for evaluation, and the evaluation itself will be documented, and it may include, but will not be limited to achievement of objectives, performance appraisal, etc.
REPORTING PATH:
The patient/client representative will report to directly to the Administrator or to the office manager if such position exists.
The patient/client representative may have supervisory responsibilities as indicated by the Administrator, when it is necessary, for the other clerical office staff, when the position of office manager does not exists.
WORKING CONDITIONS AND RISKS ASSOCIATED:
Back problems from incorrect posture at the desk, as well as incorrect light induced injuries of the eye, computer glare, hand problems due to the computer keyboard, electrical injuries due to handling of office electrical equipment or appliances, etc. may be taken into consideration.

Pay: $20.00 - $30.00 per hour
Expected hours: 8 – 40 per week
