Enhabit is hiring an Intake Coordinator to join our team. This is an office based role that can work from our Jupiter office. The Intake Coordinator will coordinate patient referrals and ensure adequate coordination and transition to home health services.

- Ensure all demographic information is accurate on the referral form based on payor eligibility
- Obtain authorization from the payor for services to be delivered
- Communicate with patients and their families; introduce company, discuss services to be rendered, and inform them of the potential start of care visit date
- Communicate with the billing manager to ensure verification of benefits
- Attach referral paperwork to medical record timely and ensure it is made available to field staff
- Communicate acceptance of referrals clearly with referral sources
- Must possess a high school diploma or equivalent.
- A graduate of an approved school of professional nursing is preferred; may be an RN or LVN / LPN.
- Must have at least two years of experience in the health care industry and one year experience in home health.
- Must have demonstrated knowledge of Medicare guidelines that govern a home health.
- Must have demonstrated experience in customer service.
- Experience with intake responsibilities and working with a multi-location company is preferred.
