Job Description
A Patient Navigator is a person with direct knowledge of the local community who functions as a “guide” and aids community members in “navigating” the health care system and accessing related social and financial services to improve health outcomes. The Patient Navigator performs a variety of information dissemination, health education, clerical and medical support services within the clinic setting and in the community. A primary function of the Navigator Is to establish and maintain communication between patients, their families, care team members, and the health care and social service systems to help facilitate the care of individuals.
Reports to: Outreach and Enrollment Manager
1. High school diploma or GED. Previous experience in a health care setting is a plus.
2. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
3. Good written and verbal communication skills.
4. Ability to be trained to perform a variety of tasks.
5. Must have direct knowledge of the local community.
6. Strong interest in the community and its people.
7. Committed to ongoing learning, able to model the behavior and activities that will support both individual development and community capacity-building.
8. Possess communication and other interpersonal skills that can ensure an ability to collaborate with local people and organizations.
9. Fluency in English (speaks, read, and write).
10. Fluency in Spanish (speaks, translate, read, and write).
11. Ability to maintain absolute confidentiality about health care and other patient/client information.
12. Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
