Under direction of the Staff Services Manager I in the Patient Management Unit's (PMU) Post-Trial Programs Case Management, the Lead Patient Navigator (HPS I) will serve as a highly skilled technical specialist and project manager, working on identifying patient treatment pathways for the post-trial patient populations, such as the Incompetent to Stand Trial (IST) patient population. In this role, the individual will work closely with internal and external stakeholders to ensure timely and appropriate treatment for each patients' individual needs. The incumbent will maintain consistent communication with assigned counties to build strong collaborative relationships that will ultimately lead to the identification and resolution of any treatment, transportation, and/or communication barriers. This individual will serve in a lead capacity for requests from the Department of State Hospital's (DSH) Legal Division, the PMU Care Coordination Clinical Team, the various treatment facilities, and other key stakeholders. Conducts research, prepares reports, and relays information pertaining to special project assignments, as requested.
The HPS I shall exercise a high degree of independence, navigating special projects as requested and serving as the Subject Matter Expert over their assigned workload in PMU.
This HPS I will serve in a lead capacity addressing requests from the Department of State Hospital's (DSH) Legal Division, the PMU Care Coordination Clinical Team, and other key stakeholders. Additionally, they will conduct research, prepare reports, and relay information pertaining to special project assignments, as requested by management.
The HPS I shall exercise a high degree of independence while serving as the subject matter expert over their assigned workload in PMU. This role requires impeccable customer service and strong analytical skills, consistently providing courteous and professional coordination with county partners, DSH staff across facilities, contractors, as well as external stakeholders. Incumbent should have intermediate to advanced skills with Microsoft Excel.

Working Conditions
- DSH-Sacramento is located at a new 11 floor high rise building.
- This position is physically located in Sacramento and is currently offered with the potential for a hybrid work environment. Consistent with Policy Directive 5338, Telework may be considered with prior approval from management. If approved, incumbent is expected to report to work at the Allenby building at least twice per week, as directed by their supervisor. Travel throughout California site visits, including overnight travel, to DSH partnered treatment facilities will be required with prior notice.
- Incumbent must comply with Government Code Chapter 3 State Employee Telecommuting Program [14200-14203] (Chapter 3 added by Stats. 1990, Ch. 1389, Sec. 1.) that disallows California state employees to reside out of state while performing their work duties.
