General Summary:
The Patient Navigator functions as the primary care coordinator for patients and collaborates with key clinical staff to ensure comprehensive care and treatment. Coordinates appointments, provides logistical support, acts as a liaison between the patient and medical team, directs patient inquires to appropriate staff, and facilitates collection of medical records, insurance and financial information. Position includes direct contact and communication with patient, in person, on telephone or via e-mail.
Essential Functions and Responsibilities:
Individual:
- Educates the patient on the purpose of the visit and schedules the appointment.
- Oversees the gathering of patient information and records from healthcare providers in preparation for appointment.
- Enters patient demographics including need for interpreter services into appropriate patient scheduling system. Contacts patient in advance to confirm appointment.
- Ensures that subsequent medical information is added to patient chart as necessary.
- Arranges for interpreter services for all appointments and treatments as needed.
- Tracks consultation, simulation and new appointments to be made available to patients as needed.
- Coordinates multi-site appointments and schedules ancillary appointments as requested. Refers patient to appropriate staff regarding advance directives, community resources, transportation and insurance issues.
- Facilitates access to medical team and informs physician of status changes or obstacles to patient treatment compliance.
Team:
- Assists in maintaining safety standards within the clinic.
- Maintains and helps to ensure that all documentation is in a state of audit-readiness.
- Promotes ongoing communication and collaboration with the clinical manager, physicians, and appropriate staff to ensure patient safety and adherence to clinic protocols.
- Communicates with internal and external referrals to ensure prompt patient access.
- Takes initiative to identify and provide solutions to work-related problems, continuously seeking ways to improve services.
- Updates Clinical Manager of changes affecting their schedule.
Practice:
- Professional representative within the clinic and the community.
- Participates in clinic meetings as required.
- Demonstrates positive initiative and cooperation between departments.
- Other duties as assigned.
Qualifications:
Education:
- High school diploma or GED required.
- Graduate of an accredited baccalaureate school in a healthcare-related field preferred.
Experience:
- Experience working in the ambulatory clinic healthcare setting.
- Ability to work independently in a fast-paced, demanding environment with minimal supervision; ability to manage and oversee multiple tasks simultaneously, including high daily call volume.
- Excellent oral and written communication skills.
- Diverse and culturally sensitive customer service skills.
- Ability to manage high-stress, chronically ill population with complicated psycho-social issues.
- Computer skills- Microsoft Word, Excel
Preferred Qualifications:
- Prior experience leading groups of clinical and non-clinic staff for care coordination.
- Experience working with healthcare payor systems.

The selected candidate's compensation will be determined based on the individual's skills, experience, internal/market equity factors, and qualifications. This posted minimum and maximum range represents the minimum and maximum of what we reasonably expect to compensate for the position. Furthermore, all compensation decisions are ultimately determined in accordance with our compensation philosophy. Compensation for positions covered by collective bargaining agreements are governed by the agreements in the aforementioned document.
