Job Description
The role of an Intake Coordinator is one that requires a great deal of compassion and patience. They are responsible for interacting with patients who are seeking care from Home Health agencies, as well as their families and loved ones.
In this role, they must be able to listen carefully to the needs of these individuals while also ensuring that all relevant information is properly documented in an organized fashion.
Home HealthIntake Coordinator Job Duties
Home health Intake Coordinators have a wide range of responsibilities, which can include:
- Interviewing patients to gather information about their health status, living situation, social support system, and goals for care
- Assessing patients’ needs by reviewing medical records and other documents
- Coordinating patient care with nurses, therapists, social workers, and other members of the healthcare team
- Explaining the nature of Home Health services to patients, families, and caregivers
- Scheduling home visits with patients in order to review their progress, administer treatment, and offer support
- Reviewing patients’ insurance coverage to determine whether they are eligible For Home Health care services
- Recommending nursing services to patients who need skilled nursing care on a regular basis
- Providing information to patients about their condition and treatment plan, including instructions on how to take medications or perform exercises at home
- Coordinating with physicians, therapists, and other healthcare providers regarding patient care plans
*LVN Background is plus.
*MA Background is a plus.
*Spanish Speaker is a plus.
*One Year of experience in Home Health.
