Advocate Program by working closely with the youth and family to provide advocacy and support towards targeted goals.

Responsibilities
The Treatment Advocate will perform duties including, but not limited to:
- Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
- Participate in the development of the youth treatment/service plans with the client, caregiver(s), referring agency, caseworker, significant family member(s), and natural support.
- Identify resources and services in the community that may not be available through the internal service continuum.
- Assist in family reunification efforts with the youths’ primary families or other permanent living arrangements consistent with the youth’s case plan.
- Assist with completing intake documents for newly placed youth.
- Follow up with DFCS staff to ensure that intake documentation has been received and completed and signed as per state guidelines.
- Assist in completing initial visit’s for newly placed youth and weekly contacts for youth who are in there first 90 days.
- Assist ongoing and as needed with weekly contacts for MAX, MAAC and SMWO youth.
- Assist Parents with Coordination of Initial EPSDT Appointments.
- Assist with monitoring/ providing transportation for foster parents and youth for crisis and emergency respites/placement changes.
- Assist in coordinating respites for foster parents and youth
- Assist with meeting 24 hour contact requirement with youth where a CPS report has been made including gathering statements and completing documentation for PVA
- Maintain client contact as required by program expectations.
- Actively and directly engage with the youth to meet service needs and goals/objectives of the treatment/service plan.
MINIMUM QUALIFICATIONS
- 21 years of age or older.
- High School Diploma or equivalent GED. Associate’s Degree in social services is preferred.
- Reliable transportation.
- Driver’s License with a safe driving record.
- Working, reliable telephone.
- Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
OTHER SKILLS
- 2 years’ experience working directly with youth and families is preferred.
- Works well independently and as a team member.
- Multi-task efficiently and be flexible in all situations.
- Openness to traveling as needed.
