Job Summary

• We are seeking a Registered Nurse who will be responsible for overseeing and guiding a team of Chronic Care Managers to ensure the delivery of high-quality, patient-centered care for individuals with chronic health conditions.

Responsibilities and Duties

Supervision and Leadership:

• Assist with recruiting team of Chronic Care Managers - including interviewing, training, onboarding new hires
• Assist with drafting and implementation of Chronic Care management clinical protocols
• Provide leadership and guidance to a team of Chronic Care Managers
• Conduct regular team meetings to facilitate open communication, share insights, and address challenges
• Foster a culture of collaboration, continuous learning, and excellence within the team
• Utilize your expertise in Chronic Care management to support the development and implementation of care plans
• Oversee the development and implementation of individualized care plans for patients with chronic conditions
• Ensure that care plans are tailored to meet the unique needs of individuals with chronic health conditions
• Proactively identify and address barriers to effective care coordination
• Stay abreast of the latest developments in Chronic Care management and incorporate best practices into team processes
• Monitor and evaluate the quality of care provided by the Chronic Care management team
• Implement and oversee quality improvement initiatives to enhance patient outcomes
• Ensure compliance with relevant healthcare regulations and standards

Team Training and Development:

• Conduct ongoing training sessions to keep the team updated on the latest advancements in Chronic Care management
• Identify and address training needs to enhance the skills and knowledge of team members
• Collaborate with other healthcare professionals, including physicians and nurse practitioners to ensure comprehensive and coordinated care
• Establish and maintain effective communication channels with internal and external stakeholders

Data Management:

• Oversee the collection and analysis of data related to Chronic Care outcomes
• Use data-driven insights to identify areas for improvement and implement changes as needed
• Provide patient assistance through advocacy to client and family for entitlements and other community services

Required Experience, Skills and Qualifications:

• Must have a dedicated and secure home office to maintain PHI, and ability to work from home office independently with minimal disruptions
• Graduate from an accredited school of Nursing
• Current license to practice as a RN
• 3+ years of experience as a Registered Nurse
• 2+ years in a virtual care role
• Diabetes care management experience a plus
• Geriatric experience a plus
• Bilingual in Spanish is a plus*
• Outstanding clinical expertise
• Strong verbal and written communication skills
• Timely and professional
• Tech savvy with proficiency in: Internet Navigation, Smartphone technology, Microsoft Office Suite products (Word, Excel, and Outlook) is required

Job Type: Full-time

Pay: $40.00 - $45.00 per hour

Expected hours: 40 per week
