About our team
The Department of Clinical Transformation will be implementing team-based care in southwestern Michigan. Centralizing the oversight of team-based care will enable introduction of initiatives that have been successful in other regions. These initiatives include centralized triage and transitions of care, central referrals to Care Management, and embedding care mangers in offices.

Our provider panel consists of more than 130 primary care providers, and over 800 specialty care providers who are dedicated to serving the health care needs of our community. Corewell Health Lakeland includes three acute care hospitals in St. Joseph, Niles, and Watervliet, as well as multiple other state-of-the-art facilities throughout the region.
Scope of work
The Care Management Supervisor will oversee the care coordination staff. You will plan, organize, and supervise all assigned clinical and program services; ensure appropriate clinical supervision and case consultation for clinical staff including overseeing and supporting professional staff development. You will also supervise within established clinical quality standards and quality management plan protocols and evaluates staff competencies and identify training needs. A professional relationship with providers is key when cases require referral for specialized treatment or to another level of care. You must be willing and able to visit the practices in southwest Michigan where we have an on-site care coordinator.

Qualifications
  • Bachelors in nursing or equivalent in nursing Required.
  • Master's Degree in nursing Preferred.
  • 5 years of relevant experience full time clinical Required.
  • MI Registered Nurse (RN) Required or
  • MI MSW-Master
  • Certified (CCM) - CCMC Commission for Case Manager Certification Preferred.
  • AHA or ARC Basic Life Support (BLS) Required.
  • MI Driver's License Required.
