Overview:
Expected pay for this position is $37.99 - $44.70/hour. Actual pay will be determined by experience, skills and internal equity.

POSITION SUMMARY:

The Supervisor of Care Management is responsible for effectively providing daily leadership for Care Management functions at assigned Medical Center(s). Responsible for assessment, developing and implementing the care processes for care coordination, transition of care, and discharge planning in collaboration with frontline staff, team leads, facility leaders, and regional Care Management leadership. Ensures the services and programs provided by the staff achieve established process and outcome measures. Responsible for consistent application and follow through with established policy and procedure around Case/Care Management functions as outlined by Ministry Care Management. Utilizes leadership skills and competencies to create and enhance a culture that supports the Care Management. In accordance with strategic initiatives, incorporates effective change management philosophies in all performance improvement efforts. Focuses on consistent performance of staff, supports a proactive approach to outlier management, and escalation of barriers. Flexible work schedule required including weekend, holiday, on-call rotation.
Qualifications:
REQUIRED QUALIFICATIONS:

Education:

Bachelors

Specific Majors: Master's degree in social work or obtain within 5 years

Licensure/Certifications:

Shall comply with the Illinois or Michigan state licensing and certification requirements or jurisdiction(s) in which she or he practices.

Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination/apply for licensing for practice of social work.

All BSW mission partners must obtain applicable license as awarded by the Department of Professional Regulation within the mission partner's designated state (Illinois/Michigan) of practice within 4 years from date of hire with detail timeline below.

Mission Partner is required to identify clinical supervisor by day 91 of hire with a plan to complete the supervision within in 3 years Mission partner is accountable to ensure completion of state application form Once the 3 year clinical supervision is completed the mission partner must submit state application within one month

All BSW mission partners must obtain licensure within 4 years from date of hire

If a mission partner comes in with documented supervised clinical hours those will be deducted from the 4 year time requirement

Social Worker with an MSW who has not obtained their license will be required to obtain applicable license as awarded by the Department of Professional Regulation within the mission partner's designated stat (Illinois/Michigan) of practice within 1 years from date of hire with detail timeline below.

Must start licensing application process by day 91 of hire

All MSW mission partners must obtain licensure within 1 years from date of hire Licensed Clinical Professional Counselor (LCPC) license also considered.

Care Management certification for adult or pediatric as appropriate within 3 years of hire.

Experience:

3 years of experience caring for patients

Other skills/knowledge:

Excellent interpersonal and communication skills.

Solid computer skills, including proficiency with Microsoft software.

Strong analytical and problem solving skills, with the ability to be detail oriented.

PREFERRED QUALIFICATIONS:

Experience:

Experience with chronic, complex illnesses adult and/or pediatric Care Management

Other skills/knowledge:

Broad knowledge of chronic illness and medical and community resources Working knowledge of a clinical EMR system and Microsoft Office applications Interpersonal skills in order to coach, mentor, and effectively lead assigned staff, facilitate, organize, and direct the work of others and to maintain positive relationships among leadership, Ministry and facility staff, and internal and external stakeholders

Positive, empathetic approach and ability to communicate effectively to patients and families with a variety of health issues, age groups, and socioeconomic backgrounds

Proven skills in written and verbal communications, problem solving, decision making, independent judgement, creativity, prioritization and delegation

Advanced analytical skills necessary to develop, implement and administer policies, procedures; analyze financial and clinical data and prepare clear, concise reports that identify opportunities and propose appropriate action plans and budgets
