The Supervisor is a part of a multidisciplinary team and is responsible for coordination of all aspects of Enhanced Care Management (ECM). The Supervisor oversees provision of ECM services and implementation of the care plans. A Supervisor oversees the delivery of Enhanced Care Management services where the member lives, seeks care, or finds most easily accessible including in the office, and in the community. A Program Supervisor plans and supervises a program and provides a variety of administrative and program management tasks. This position implements program goals as well as carries out important operational duties.
Qualifications:
- Bachelor of Arts or Bachelor of Science degree in Counseling, Social Work, or related field preferred or licensed vocational nurse. Degree may be waived with equivalent experience.
- Knowledge of community resources preferred.
- Bilingual preferred.
- Must be 21 years old or over.
- A valid California driver’s license with less than two minor moving violations.

Responsibilities:
- Engage and enroll eligible members in ECM.
- Oversees provision of ECM services and implementation of the care plan.
- Utilize motivational interviewing, trauma-informed care and harm-reduction strategies.
- Responsible for coordination of services to ensure a seamless experience and unduplicated services.
- Advocate on behalf of members with health care professionals.
- Connect members to other social services and support including transportation.
- Provide Community Outreach and attend Community Events.
- Provide health promotion and self-management training.
- Accompany members to office visits, as needed.
- Monitor treatment adherence
- Coordinate with hospital staff on discharge plans.
- Attend all required meetings.
- Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
- Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
- Assist with interview process.
- Assist with new employee training.
- Other duties as assigned.
- Responsible for preventing and controlling infection.
- Responsible for maintaining a culture of quality and safety.
- Effectively use de-escalation skills and techniques for crisis prevention.
- Properly administer the C-SSRS to help prevent suicide.
