Job description

We are seeking an experienced child services professional with the ability to provide high quality leadership and crisis management with little to no assistance; and must be competent in using effective communication and problem solving skills.

Company Overview:

Safe Haven Community Services is non-profit 501© (3) organization, licensed and funded by the Texas Department of Family and Protective Services since 2005, which operates as a Foster Care and adoption agency for foster and/or adopt children specifically with special needs.

Job Description:

The Foster-Care Coordinator is the anchor of the overall Case Management service team, by performing assigned lead work duties over case managers providing on-the-job training and technical support, monitoring child services for compliance in the application of program policies and procedure, HHSC rules and regulations, DFPS contractual performance outcomes. In addition to identifying areas for process improvement to support the highest quality delivery of Case Management services to every child in care.

Must be a US citizen and have a cleared CBC and FBI clearance, including good organizational, time management, and communication skills, as well as the ability to work independently. Must possess a valid Texas Driver’s License and the ability to use private car to transport self and others, and must comply with the state policy concerning minimum car insurance coverage.

Education:

A Master’s degree from an accredited college or university; and Nine credit hours in undergraduate or graduate level courses that focus on family and individual function and interaction.

A bachelor’s degree from an accredited college or university in social work or other human services field; and Nine credit hours in undergraduate or graduate level courses that focus on family and individual function and interaction.

Two years of documented full-time experience in a residential child-care operation, or as a conservatorship caseworker or foster adoptive home development worker for the department. The experience must be in conducting assessments, service planning, or Case Management duties.

ESSENTIAL DUTIES & RESPONSIBILITIES

These activities require a comprehensive understanding of complex program rules, regulations and standards, and the ability to analyze unique situations.

Coaching, Training, and Technical Support Duties: (15%)

· Performs lead work duties over case managers, maintains a thorough and updated knowledge of all DFPS and HHSC standards and guidelines, interprets and explains rules and regulations to case managers, and provides assistance with managing team workloads

· Provides on-the-job training, mentor new and established case managers, and collaborates with the agency Director of Foster Care and Adoption Services on course content and materials

· Coaches and mentor less-experienced case managers on providing quality Case Management services, facilitate new employee training, shadowing of case managers, provides technical training, and assistance to the Case Management team around the established policies, procedures and protocols which includes clinical services ( i.e. admission assessment, service plan, and safety plan development, service care coordination, connection to support services);and use of the organization’s database system(Extended Reach)

· Provides work related instructions and assists the Case Management team with difficult and/or unusual assignments

· Collaborate with Director of Foster Care and Adoption Services to develop key tools; including clinical and operational protocols, assessments, workflows, identification of community resources

· Contributes to the development of the performance enhancement plan, documents performance, provides performance feedback, and furnishes information for the formal performance evaluation

· Responds with written response to informal grievances and relays information to the Director of Foster Care and Adoption Services

Quality Assurance Duties: (25%)

· Report and document all incidents to the Director of Foster Care and Adoption Services and provide QA for incident reporting documentation, in accordance with HHSC minimum standards

· Monitors work activity for each case managers’ caseloads to ensure that cases are clearly and thoroughly documented, reviews and analyzes case records and reports for completeness and accuracy, identifies compliance and production problems, and recommends and implements process improvements

· Facilitates regular team meetings with the Case Management team where case review audits, foster-adopt home inspection findings, and new processes are discussed. Listens to feedback, brainstorms solutions and action plans and provides recap to Case Management team as needed

· Proactively identify case managers and foster-adopt parents who may need additional support and training, and works with the Case Management team and Foster Home Compliance Specialist team to conduct root cause analysis, develop solutions, and implement action plans

· Resolves problems encountered during daily operations and determines appropriate solutions in the absence of the Director of Foster Care and Adoption Services

· Continuously identifies and surfaces challenges and best practices from the Case Management team, and works with the Director of Foster Care and Adoption Services to improve operations (protocol)

Meetings and Support Services Duties: (20%)

· Initiate, develop, facilitate individual and group training for Case Management team as needed

· Initiate, facilitate quarterly foster-adopt parent support groups

· Participate in weekly supervision meetings with Director of Foster Care and Adoption Services

· Participate in monthly agency meeting

· Initiate, facilitate child placement preservation plan meetings

· Participate in DFPS permanency planning, court hearings, and school related meeting for the child as needed

· Initiate, facilitate plan of service meetings and reviews, collaborative intake meetings, as well as subsequent placement, respite, and discharge meetings

· Assist Director of Foster Care and Adoption Services and the Case Management team with finding inter-agency and outer-agency respite homes for children with high risk behaviors (as needed)

Child Placement Management Staff Duties: (50%)

The Foster Adopt Coordinator must review, comment, provide training, approve and assistant to subordinate staff complete the follow tasks:

· Assessment/Admission forms

· Initial and subsequent placement documents

· Initial and update ISP

· Child Progress Notes and Case Notes

· Discharge or transfer plans and summaries

· Any restrictions imposed on the child for more than seven days that the treatment director or service planning team have not approved; and that continues for more than 7 days and must be re-evaluated by the CPMS

· Any restrictions to communication and visitation with family imposed on a child

· Any restrictions to a particular room or building for more than 24 hours imposed on a child

· Other agency documents completed by case managers described in SHCS policies and procedures

· Reviews and approves all investigation findings of minimum standards deficiencies that Licensing requires Safe Haven to conduct

· Investigation reports of minimum standards deficiencies that Licensing requested the agency to conduct

· Child placement staff contacts with children as outlined in 749.1291.

Case Management Duties: (5%)

· Places a child into an foster/adoptive home or other substitute living arrangement

· Maintaining direct contact with the child and the foster/adoptive parents or other caregivers in the absence of the assigned case manager

· Maintain appropriate boundaries between field professional, foster-adoptive parents, case managers and interns regarding personal issues

· Respect the dignity and worth of all foster-adopt parents

· Maintain confidentiality per agency standards and all applicable codes of ethics

· Consider the environmental, cultural, safety, diversity issues and needs for the child and foster-adopt parent

· Identify and understand the strengths, abilities and competencies of the foster-adopt parent

Transportation Services Duties: (1%)

· Transport the child to sibling and family visits, when necessary to ensure that the child maintains family connection

· Transport the child to non-emergency medical, dental, psychiatric, psychological, and other service appointments, when necessary to ensure the child’s health and well-being is maintained

Tracking Out Comes for Children in Care: (2%)

· See DFPS Residential Child Care Contract

· Review and verify all PMET Monthly Data prior to submitting to the Director of Foster Care and Adoption Services each month

· Explore solution to correct and improve on outcomes for children as needed

Professional Development: (2%)

· Use and follow a professional development plan to address own strengths and limitations in periodic and annual reviews

· Attend scheduled in-service training in order to develop professional skills

· Meet with immediate supervisor on a semi-annual basis in order to review performance, accomplishments, challenges and job descriptions

· To be a flexible member of the agency's team in order to accomplish the overall agency goals

Supervision with Immediate Supervisor: (2%)

· Communicate regularly with immediate supervisor and seek supervision when necessary

· Be prepared to address appropriate issues and challenges, including sharing your solutions during supervisory conferences and meetings

· Accept and implement direction, instructions and corrections from immediate supervisor

· Complete other job related duties as assigned by immediate supervisor

· Supervises child placement staff (case managers), if any including planning for the child placement staff’s professional development and taking any other appropriate action in regard to their child-placing decisions in conjunction with the Director of Foster Care and Adoption Services

Pay: $50,000.00 - $56,000.00 per year
