Job Description
The Care Manager Supervisor is a multifaceted role. The Supervisor will assist in developing and guiding the program. The primary responsibilities of the Care Manager Supervisor are to provide clinical, administrative, and operations supervision and guidance to the Care Management team to promote whole person, integrated care by planning, coordinating, tracking, and monitoring care to individuals to achieve the key goals of the behavioral health tailored plans. Collaborate with other agencies to ensure team-based care. Supervisors will not have a caseload, but will supervise the care management team, provide coverage for vacation, sick leave, and staff turnovers.
Qualifications/Education/Experience: A master’s-level fully Licensed Clinical Social Worker (LCSW), fully Licensed Clinical Mental Health Counselor (LCMHC), fully Licensed Psychological Associate (LPA), fully Licensed Marriage and Family Therapist (LMFT), or licensure as an RN; and three years of experience providing care management, case management, or care coordination to the population being served.
