Transitional Care Coordinator - Patient Intake Coordinator
- Location: Fullerton, CA
- Work schedule: Monday to Friday, 8:30 AM to 5:00 PM.
- Reports to: Chief Nursing Officer
- Ideal start: ASAP
- Hourly compensation range: $25hr-$30hr (Based on Experience)

Position Summary
Under the direction of the Transitional Care Nursing (TCN) supervisor and VP of Clinical Operations, the Intake Coordinator works with Transitional Care Team on site in facilities.
Your Responsibilities
- Obtain daily census
- Assist with checking patient daily updates (still in facility, discharged, etc.)
- Intake/patient registration
- Verify patient Medicare eligibility
- Arrange patient packets as needed
- Visiting patient on discharge date to give packet and finalize discharge disposition and tie up loose ends and communicate it with TCN
- Other duties may be applied
Your Qualifications
- Experience in one or more of the following areas preferred: Acute care
- Community health, Ambulatory health, Skilled or rehabilitation nursing, Doctor’s office
- Current AHA-approved BLS
- Strong computer literacy skills (i.e. Windows, Microsoft Office applications, mobile apps)
- Reliable transportation
