Position Title: Patient Care Coordinator
Reports to: Front office Manager
POSITION PURPOSE
The Patient Care Coordinator is responsible for the current and follow up care for patients that are seen within our office to ensure we meet their needs and expectations while in the office. They are to schedule follow up appointments, procedures and annual visits as directed by providers or patient request via in office or over the phone. Patient Care Coordinators are responsible for collecting the amount due for their care.
DUTIES AND RESPONSIBILITIES
CHECK IN & OUT
- Greets and checks patients in upon arrival
- Presents forms to be filled out, provides any assistance needed by patient.
- Copies driver’s license and both sides of insurance cards
- Enter demographic information into Nextech for new patients
- Activate patient portal meeting MIPS requirements
- Provides copy of Practice Financial Policy to patient for reading and signature. Assists the patient in understanding of the financial policy. Explains rules of patient’s insurance plan when necessary.
- Checks demographic information with established patients and enters any changes in Nextech
- Make follow up appointments according to fee ticket
- Give handouts to patients as needed for more information on procedures if one is to be scheduled
- Collect any fees due via Health iPass. This includes copays and deductibles for patients for whom insurance will be filed, and total cost of the visit for those who will file their own claims and those who have had non covered services rendered.

- Return patient emails promptly
- Include your professional office signature
- Follow HIPAA policy when emailing patients
TELEPHONES
- Answer phone by third ring “Thank you for calling Texas Dermatology, this is Mary, how may I help you.”
- Triage patient phone calls – please see phone tree diagram
- Return patient calls & emails as directed via shifts
- Enter demographic information into Nextech - new patients including address, phone, email & referring provider if any
- Verify existing patient information in Nextech – all demographic & insurance information
OTHER
- Treats all patients with respect, is polite and professional, and keeps patients informed of any delays in being seen.
- Always maintains patient privacy.
- Maintains orderliness of reception room throughout the day, paying attention to the neatness of magazines and removing items discarded by patients.
- Creates a system for inventorying and ordering office supplies, fax, copy, and printer supplies and toners, and charts and related supplies.
- Assists in the training of new Front Desk employees.
- Handles mail and courier reports as directed by the manager and/or physician.
- Additional duties as assigned by the practice manager or the physician(s).
PHYSICAL/MENTAL DEMANDS: Varied activities including standing, walking, reaching, bending and lifting. Requires full range of body motion including handling of patients, manual and finger dexterity and eye-hand coordination. Must have the skills to be able to use a variety of office equipment successfully. Must be able to handle stressful situations. Must possess mental acuity for attention to accuracy and detail.
