Job Description
We are seeking a motivated, and experienced community-based Nurse to provide services to Medicaid members enrolled in our home and community-based programs. This position is mainly no hands-on clinical services, with no nights, weekends, or holiday shifts. This position is ideal for a nurse with home and community-based experience in the rehabilitation field of the elderly and people with disabilities desiring to live independently in the community.
RESPONSIBILITIES
- Complete and coordinate all applicable clinical assessments and clinical evaluations.
- Develop and review on an ongoing basis each member’s plan of care.
- Review the physician orders, medications and diagnosis.
- Monitor the health status of all members to ensure that all ordered services are properly delivered.
- Report changes in the health status of any member to the member’s primary physician.
- Review direct care aide and caregivers as directed.
- Conduct on-site home visits with each member enrolled member in accordance with the visit cycle policy.
- Complete a progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
- Submit semi-annual health-status reports to the member’s PCP
- Plan for and implement discharges and transition from the agency programs
- Conduct an orientation for each Direct Care Aide or caregiver before they can begin care provision.
- Provide ongoing training to Direct Care Aides and caregivers on health and aging
- Contact with members primary care physician to obtain medical information and physician clearances
- Other responsibilities as assigned by the Agency.
REQUIREMENT:
- Must be a graduate from a School of Nursing.
- Must be licensed in the State as a Registered Nurse or Licensed Practical Nurse LPN.
- Ability to travel throughout Massachusetts, mode of transportation negotiable
- Minimum two years of experience working with elderly or disabled individuals.
- Excellent organizational, problem solving/planning and communication skills
- Demonstrated skills in Windows, Microsoft Word, Excel, Access and Outlook.
- Must be able to contribute to the quality of care being rendered through constructive communication with managers and staff.
- Must have a criminal background check
- Must have a current CPR certification.

Pay: $40.00 - $45.00 per hour
