School Based Community Health Worker Manager- Community Support Program

In this role you will:
The School Based Community Health Worker (SBCHW) Manager at the Lynn Community Health Center (LCHC) is part of an integrated and interdisciplinary care team working to address the needs of students in Lynn. The SBCHW Manager will provide culturally appropriate individual and group health and resource education, informal counseling, case management, care coordination, and advocacy in order to promote, facilitate and improve access to ongoing primary and behavioral health care.
BASIC QUALIFICATIONS:
- High School diploma or GED
- Graduate of MA Board approved CHW Education Training Program or willingness to complete such program within 12 months of starting in the position (reimbursed by LCHC)
- Ability to work with and relate to individuals and families from diverse backgrounds and experiences
- Strong knowledge of local community, social service, and cultural resources
Strong organizational, interpersonal, attention to detail, time management, multi-tasking, and problem solving skills
- History of good attendance and positive work attitude
- Excellent communication skills and ability to set personal boundaries
- Ability to establish positive and supportive relationships with patients and colleagues
- Ability to work independently and collaborate with a team
- Ability to motivate others and thrive in a fast-paced environment
- Ability to develop, adapt, and execute outreach plans
- Excellent English written and verbal skills
- Computer literacy and ability to document appropriately in electronic health record
- Must have a valid driver’s license and transportation

EXPERIENCE AND TRAINING:
- Three or more years experience as a Community Health Worker or case manager in a Community Health setting; and/or related experience in health/human services, social services or advocacy.
- Experience in School Based outreach within mental health, community-based treatments or medical comorbidity.
- Experience providing health education and knowledge to youth/ teens
- Experience navigating systems of care and community agencies in Essex County
DESIRED QUALIFICATIONS:
- Bachelor’s degree in health sciences, social work, sociology, human services or related field
