General Responsibilities:
- Assist the DPCS in day to day clinical operations. Assigns, schedules and coordinates patient care and assessments to the appropriate nurse, taking into consideration caseload availability, geographic areas, staff skill level and expertise
- Assist clinical staff in carrying out patient Plan of Care
- Triage clinical phone calls from patients, families, and different clinical facilities..
- Prepares reports and documents for use at IDG
- Monitors route sheets and assures prompt return of documents and records.
- Keeps track of certifications, re-certifications, physician orders, assessments, and logs as required.
- Maintains patient and employee confidentiality
- QA notes as needed
Ideal Candidate Background:
- Licensed as an LVN OR RN required.
- Previous experience in hospice required
- Bilingual is preferred.
- Must have excellent interpersonal skills and a passion for hospice.
- Ability to work with little supervision.
- Attention to detail, vigilance, poise, and composure in high stress situations.
Job Type: Full-time
Pay: $30.00 - $35.00 per hour
Expected hours: 40 per week
Schedule:
- Monday to Friday
- No weekends
