Position Summary:

Reporting to the Vice President of Quality, this position coordinates and assists other health team members in providing care for patients with disorders of the integumentary gastrointestinal, and genito-urinary systems. This includes assisting staff and physicians in managing the care of patients with complex pressure injuries, draining wounds, fistulas, ostomies, and incontinence. A key role for this position also includes direct patient/family education as well as educating, coaching and providing resources to other health team members as they educate patients/family/significant others in patient care management and related discharge planning.
  • Performs a physical assessment on the patient and helps nursing staff to identify priority care needs for the patient with: • altered skin integrity (including pressure injuries, fistulas, draining wounds) • altered gastrointestinal functioning (e.g. ostomy, GT feeding tube • altered genitor-urinary functioning (e.g. suprapubic or other urinary catheters) This includes assessing physical, emotional, social and spiritual factors that may affect the patient’s rehabilitation. Assessment is done through direct observation, communication with the patient, family, and other members of the health team, and review of the patient’s medical record.
  • Initiates plans for continuity of care in collaboration with the physicians, nursing staff, social worker and other health care team members. Demonstrates skill in the nursing process and critical thinking.
  • Assists staff with implementing the patient care plan by performing specialty-specific independent nursing actions. Performs therapeutic measures as delegated by a physician, or as otherwise needed, within own scope of practice.
  • Documents in a clear, complete, legible, organized and timely manner. Maintains accurate records and statistics. Communicates confidential and patient-sensitive information through appropriate medical center channels.
  • Provides written or verbal instructions/reports to agencies or persons responsible for a patients’ care after discharge related to own specialty area, as needed.
  • Verbalizes and demonstrates knowledge of clinical information and practices essential to providing age-specific care for the infants through the geriatric population. This includes but is not limited to: Identifying the need for additional safety measures; recognizing normal and abnormal physiological values/readings; physically assessing skin integrity, behavior, motor skills and/or activities that place patients at risk for injury; and Communicating effectively in a clear, concise, understandable, and age-appropriate manner.   • Conducts formal and informal beside and classroom instruction for physicians, nurses, students, other members of the health team and the community in the care of patients with pressure injuries, draining wounds, fistulas, stomas and incontinence.
  • Evaluates skin care and stoma products and the availability of supplies on a continuing basis. Demonstrates responsibility for cost containment, conservation of supplies and equipment, as well as other organizational resources.
  • Enhances own professional growth and development through participation in educational programs, nursing leadership meetings, hospital wide committees (e.g. Product Standards, Infection Control, Quality): maintains currency and participates in learning activities that expand competence within own specialty.
  • Demonstrates effective communication methods and skills, using lines of authority appropriately. Cooperates with other personnel in a positive manner to achieve departmental and interdepartmental objectives and maintain good employee relations.
  • Supports and adheres to established departmental policies and procedures, objectives, safety and infection control standards, including compliance with punctuality, overtime, body mechanics, TB screening, annual mandatory educational updates, attendance, English language, ID badge, and dress code standards.
  • Coordinator for the facility’s Core measure other clinical and operational process improvement initiatives as assigned. Performs detailed, concentrated clinical and regulatory review with limited supervision.
  • Identifies, at the earliest opportunity, any regulatory changes that may impact the organization and make recommendations of how compliance can be achieved.
  • Uses problem-solving skills to proactively take action on individual cases not meeting measure requirements.
  • Serves as a clinical quality resource for nursing and other clinical personnel, physicians, and other ancillary departments.
  • Prepares reports for various stakeholders as assigned. Able to tailor data and reports to intended audience with minimal supervision.
  • Communicates relevant information to appropriate department managers, provides follow-up to ensure new standards are being met. Tracks elements of action plans to ensure timely monitoring and prepares reports as requested.
  • Communicates and interacts with physicians professionally and effectively.
  • Ensures the integrity of data being reported and utilized.
  • Designs and develops dashboards, presentations, educational materials relevant to the clinical audience.
  • Demonstrates clinical and professional judgment in case referral determination.
  • Maintains an accurate knowledge of clinical quality measures and the outcomes to be achieved.
  • Provides assistance and support for patient care tracers as assigned.
  • Exhibits flexibility to learn new areas, expanding clinical, professional, regulatory and technical knowledge and skills.
  • Abstraction and analysis mandatory outcome measures, as necessary.
  • Assists the Quality Department in achieving identified quality outcomes.
  • Collects, analyze, and inputs clinical and operational data into various databases to support process improvement. Works collaboratively with teams to set goals that are realistic and meaningful and result in quantifiable improvement.
  • Ensures an effective data collection and processing system by reviewing and analyzing data input, processing and data output activities.
  • Ensures accuracy and consistency in data collection by the Quality Management staff.
  • Performs audits to evaluate the quality and timelines of medical records completion to ensure optimum care delivery.
  • Assists with special projects, committees provides meeting support, e.g., ensure meetings are scheduled and designated individuals invited, prepares agendas, records/distributes/archives minutes, ensure follow-up on action items when needed.
  • Maintains accurate minutes and communication to various stakeholders within the organization.
  • Produces reports, completes assigned projects timely with little follow up.
  • Communicates and interacts with physicians, administration, and peer groups, appropriately, professionally and effectively.
  • Provides assistance and support for patient care tracers as directed.
  • Exhibits flexibility to learn new areas, expanding professional, regulatory and technical knowledge and skills.
  • Maintains confidentiality of information.

Minimum Education:
  • Graduate of an Associate Degree or Diploma Nursing program.
Preferred Education:
  • BSN Preferred.
Minimum Work Experience and Qualifications:
  • One year in medical/surgical or specialty area.
  • Ability to communicate effectively verbally and in writing.
  • Must be able to work in a union environment.
Required Licensure, Certification, Registration or Designation:

  • Completion of an accredited Enterostomal Therapy Training program, WCN, WOCN, WCC, CWS, CWCN or CWOCN certification required.
  • Current CA RN license.
  • Current BLS certification.
  • Current Los Angeles County Fire Card required (within 30 days of employment).
