Position Summary
- The Dignity Health Stroke Coordinator is responsible for coordinating and implementing services and activities related to the Stroke Services Center.
- This position functions as a liaison between physicians, nursing staff, allied health-care professionals, patients and their families to coordinate and facilitate the care of the stroke patient throughout the continuum and to ensure the clinical care path is followed as appropriate.
- The Stroke Coordinator is responsible for planning, developing and implementing Stroke
- Center initiatives including quality, safety, and service and efficiency strategies into the plans, policies, and organizational processes that affect the Stroke Center's performance metrics and certification.
- The Stroke Coordinator will maintain all requirements of the Joint Commission (TJC) Disease Specific Certification for Primary Stroke Centers, Clinical Practice Guidelines and American Heart Association “Get with the Guidelines” program.
- The Stroke Coordinator participates in program development as it relates to the Stroke Program. This includes, but is not limited to staff education, community education and marketing.
Minimum:
- Five years Medical/Surgical, ED or ICU experience.
- Two years nursing management/leadership/education.
- Experience in physician relations and patient experience/satisfaction.
- Bachelor’s Degree - BSN in progress (minimum)
- Must possess and maintain CA RN License
- Flexible schedule/hours
- Computer literate, Microsoft Office, Cerner
- Must possess and maintain NIHSS Certification
Preferred:
- Bachelor's degree completed
- National certification related to Stroke Care
