Job description

JOB SUMMARY:

Oversees the operation of the Resource Center. Serves as the coordinator in the provision of service transactions and human service assistance to meet immediate client needs. Handles assessments, crisis intervention, placements, and coordination of program activities. Coordinates planning for Resource Center educational and outreach activities. Acts as the primary community point of contact for questions and information about the Resource Center services and programs.

ESSENTAIL POSITION FUNCTIONS:

  • Develops and evaluates existing and new program services based on changing needs of clientele; develops programs and secures resources to assist in the stabilization of homeless and low-income individuals and families.
  • Conducts assessments to identify program's strengths and weaknesses and to specify program goals.
  • Serves as coordinator/liaison to other agencies to obtain and coordinate informational community resource assistance on client's behalf.
  • Builds and maintains professional working relationships with community agencies and sets up Facility Use Agreements with agencies.
  • Plans, coordinates, and/or facilitates life skills classes or educational workshops to help clients gain competencies that enable them to gain upward mobility or cope with their environment.
  • Interviews applicants to determine eligibility for program services based on guidelines and requirements. Conducts need assessments, obtains pertinent information, and establishes program services for eligible clients and refers applicants to other agencies if not appropriate for program.
  • Manages incoming telecommunication to ensure clients are directed to appropriate resource services within the program, with other Social Services programs, and when appropriate refer to community agencies.
  • Prepares and maintains case records and/or logs on all participants ensuring accuracy and completeness. Enters pertinent information into the Homeless Management System (HMIS).
  • Maintains all statistical service/program data concerning the Resource Center program services. Records and submits daily and monthly statistical data as assigned. Prepares & maintains records of services and activities provided; compiles records & provides an accurate monthly report reflecting all Resource Center program activities.
  • Refers Resource Center program participants to appropriate Corps programs and/or other community agencies based on identified needs. Coordinates additional wrap around services provided by community partners.
  • Attends community meetings with related relevant agencies for networking and educating them about our services and programs.
  • Assists in the grant-writing process for additional program/service funding associated with the Resource Center program. Ensures compliance of all requirements outlined in grant agreements and/or funding contracts associated with the Resource Center Program.


EDUCATION AND EXPERIENCE:

Bachelor's degree from an accredited college or university in Social Work, Human Services, Education, Public Health, or a related social services field,

and

Two years progressively responsible experience performing social services work with at least one year in a supervisory capacity,

or

any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

LICENSES AND CERTIFICATIONS:

Valid State Driver's License

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of practices and principles of social service case management.
  • Knowledge of social services resources and agencies in the community.
  • Knowledge of effective communication and motivation practices.
  • Knowledge of Microsoft Office applications.
  • Ability to develop and evaluate the progress of completing program plans and goals based on client needs.
  • Ability to professionally interact with community agencies, community resources and with clients.
