The Patient Care Coordinator is an essential part of Rock Hill Eye Center’s team. This position provides patients with exceptional care.
Duties include:
· Answer calls regarding medication refills from patients and pharmacies.
· Investigate the status of medication refills.
· Ensure accuracy in documenting medication refills within patients’ medical record.
· Checks fax machines and EMR for patient medication refill requests faxed from pharmacies.
· Retrieves phone messages from dedicated phones for medication refill requests.
· Presents refill requests to the provider in a timely manner.
· Fax back signed medication refill requests to the identified pharmacies.
· Scan faxed signed document into patient’s chart.
· Document refill in patient’s chart.
· Notify patient of denied refill requests.
· Prepare prior authorizations for intake medications per doctor’s orders.
· Triage patient calls regarding emergency issues and assist in scheduling appointments.
· Completing medical records requests from patient and outside sources.
· Responsible for assisting patients to schedule an appointment when referred to Rock Hill Eye Center.
· Responsible for assisting patients in scheduling appointments for outbound referrals.
