Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job Description
The intake specialist is responsible for completing client intakes at agency facilities and assisting case
management/medical staff with client care needs.
The pay range for this position is $24.00 - $27.00 per hour, depending on experience. This position will assist at both our Colton and Riverside locations.
Responsibilities
- Complete the client's intake process in its entirety in a timely manner and in accordance with agency workflow and standards.
- Ensure that all compliance contracts are signed.
- Utilize the Homeless Management Information System (HMIS) to enter client information.
- Enter the client's medications into IF Database.
- Maintain internal agency documentation requirements in agency EHR and Microsoft Suites.
- Ensure that the DHS client’s are checked into Champ/HMIS on the intake day.
- Demonstrate a strong understanding of MediCal and Medicare insurance and utilize this knowledge to refer clients to insurance-funded programs that assist with social determinants of health
- Submit ERC referrals on the intake day
- Assist case managers with client service connections
- Assess client need for recuperative care services
- Assign client rounds in our EMR system.
- Establish a new PCP for intakes that are not connected
- Assist the client with making PCP and Specialist appointments as needed.
- Enter ICD10 coding into EMR system
- Reviewing referral and admission paperwork upon intake
- Assess the client's need for recuperative care services

Qualifications
Required:
- Valid CA Driver’s License and eligibility for company vehicle insurance.
- High School Diploma or equivalent.
- Customer and client relations skills
- Proficiency in Google Suite (Mail, Docs, Sheets, Calendar), Microsoft Office Suite (Word, Excel, Outlook, Teams)
- Understanding of MediCal, Medicare, and continuum of care homeless services
- Knowledge of medical coding, specifically ICD10 coding
Preferred:
- 2+ years working in homeless services preferred.
- M.A. or EMT certification
- Bilingual in English and Spanish.
