Job Overview
This is a union position.
Women age 18 and above with severe substance use disorders are able to enter residential treatment with their children under the age of 10 . In addition to treatment of severe substance use disorder, the residential program is able to serve the following specialty populations: pregnant and postpartum women, women with histories of trauma and/or domestic violence, women with co-occurring mental health disorders, women with history of traumatic brain injury, women with open DCFS cases, and women with criminal justice involvement.
The Intake Coordinator II is the first point of care for engaging clients in treatment at Prototypes treatment programs. The Intake Coordinator II is responsible for the initial assessment of candidates to determine medical necessity, completion of program admission forms, initial Treatment Plan, and urgent referrals in regards to medical and mental health needs. The Intake Coordinator II works in conjunction with Supervisor and Intake Specialist I to provide a warm hand off to the client's treatment team, assuring new clients are stabilized and urgent needs have been addressed. The Intake Coordinator II will work closely with Counselors, Case Managers, Therapists, and other team members to coordinate and monitor client issues. Often, the Intake Coordinatro II works in a group environment.
Key Responsibilities
- Provide comprehensive initial assessment of candidates' needs.
- Ensure clients' urgent needs are met and appropriate referrals have been made.
- Complete all documentation involved in admissions process.
- Begin process of assisting client in applying for Medi-Cal and/or County funding, as applicable.
- Complete Initial Treatment Plan.
- Provide a warm hand off to assigned residential treatment staff upon completion of admissions process.
- Provide follow-up to ensure services are obtained.
- Provide crisis intervention as needed within scope of practice.
- Develop collaborative relationships with community partners.
- Communicate effectively with inter-disciplinary team and participate in team meetings to review cases.
- Document relevant and required information regarding each client.
- Maintain and observe all HIPAA regulations that apply.
Education and Knowledge, Skills and Abilities
- MSW or similar Degree from an accredited program. Active registration with the BBS.
- At least two years of experience working with clients in a Mental Health, Substance Abuse, Domestic Violence and/or related field.
- Bilingual in Spanish is preferred, English and the predominant other language natural to the community where the facility is located.
- Good written and verbal skills.
- Strong Computer skills.
- Dependable automobile and insurance, registration and valid California Drivers License.
