About Homeless Children’s Network
Homeless Children's Network (HCN) works to address two needs: first, the traumatic effects of homelessness and violence in the lives of young children and their families; and second, to establish a “standard of care” across a variety of shelters and housing programs. Working directly with the community, we help children and families address the root traumas that helped shape their worldviews, explore healthier perspectives, and learn practical non-violent skill sets for managing stress. We do this through comprehensive mental health services, case management, family education and support services, and community leadership.
About the Clinical Program Coordinator Position
The Clinical Program Coordinator position is responsible for coordinating all aspects of the clinical programs across HCN and receives referrals and referral inquiries from the community, schools, and other agencies who HCN collaborates with. They determine our current capacity across programs, and then support the assignment of new clients to an appropriate clinician in collaboration with Program Managers and Directors. When we do not have current openings, they let the inquiring person know and offer any support or guidance toward other options when feasible.
Tasks include the following:
- Track clinician openings utilizing a spreadsheet shared with QA (QA tracks closures and client engagement to assist Intake Coordinators in identifying new potential openings)
- Monitor clinician capacity and productivity in collaboration with Program Managers and Directors to determine availability for additional clients.
- Complete initial eligibility checks for referrals prior to accepting/assigning (e.g. verify Medi-Cal status, ensure appropriate demographic eligibility for current opening, verify contact information has been provided)
- Support the Program Managers and Directors with the assignment of new clients to clinicians with identified openings, providing the Program Manager/Director with a copy of the referral form and any additional information provided by the referring party.
- Field and respond to capacity inquiries from the community, agency partners, and schools. When no capacity is available, provide support and guidance for alternatives when possible.
- Update the county’s system-wide capacity tracker with HCN’s current capacity each week.
Clinical Trainings/Meetings:
- Manage calendar invites for all Clinical and Consultation programs, All Staff meetings and other HCN events (virtual)
- Attend clinical related meetings and trainings - take notes and share out to team (virtual)
- Manage BBS registration renewal reminders for all clinicians across the agency (3 mo. advance notice from expiration date and 6 weeks follow up if necessary) for clinicians and update records (virtual)
- Manage and update records for onboarding, annual (renewal) and one time DPH Certification Reminders for Clinical Team (virtual)
Referrals/Eligibility
- Check MediCal for existing clients monthly and communicate with clinicians and notify Program Managers/Directors as well as clinicians if there are any changes
- Log in assigned and unassigned referrals
- Long term project: Binders organization - Digitalize, update (virtual)
Audit Support/Management
- Assist with annual site visits for different programs; manage and prep digital records and office binders on premises
- Keep track of up to date records of MediCal (re)certifications, CANS, Fire Clearance renewal, etc.
- Complete Staff Separation form for former employees, send notice to DPH (virtual) and keep copies in binder for records
REQUIRED QUALIFICATIONS
- Bachelor’s degree in relevant field, and/or qualifying experience
- Excellent oral and written communication skills
- Strong organization skills and familiarity with Excel/G-Suite
- Hybrid Remote/In-Person (Bayview district, with some meetings in Fillmore)
PREFERRED QUALIFICATIONS
- Experienced and knowledgeable in child development, especially in BIPOC families and communities
- Familiar with resources and communities in San Francisco
- Knowledge of EPSDT (Medi-Cal)
- Working knowledge/experience related to issues of homelessness and domestic violence
- Positive attitude and team play with proven ability to work with people of different intersecting identities and learning styles.
