POSITION: Employment/Community Support Specialist
DIVISION: Community Developmental ServicesDIRECTLY RESPONSIBLE TO: Case Manager

Seeking motivated Support Specialist to provide Community and employment supports. This is a full-time 33-hour a week benefitted position.
ABOUT THIS POSITION:
- Support will be provided to a 32-year-old woman totaling 25 hours per week. 10 hours per week will be in the community. Activities may include attending community events, activities with peers or attending the gym. The remaining 15 hours will be supporting her goal of maintaining competitive employment.
- Support will also be provided to a 29-year-old male totaling 8 hours per week. The support will come in the form of job coaching to assist him with maintaining employment.
- Contribute to the development of and ensure support with progress towards objectives identified in Individual Support Agreement (ISA).
- Preserve privacy, confidentiality and ensure dignity to all residents.
- Capture online notes and data related to daily experiences in electronic medical record software in a timely manner.
- Helping to promote an active lifestyle through engagement in activity and exercise.
- Participate in agency wide and position specific trainings as needed.

QUALIFICATIONS:
- High School Diploma or equivalent.
- Positive attitude and sense of humor.
- Possession of a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle.
- Ability to adapt to a changing environment.
- Prior supported employment experience preferred but not required.
