Overview
Provides administrative and secretarial support to the Care Management team and communicates and collaborates with multiple team members including RN and Social Work Care Coordinators, unit nursing unit staff, unit secretaries, and others. Exemplifies the Riverside Care Difference by showing compassion and caring to all customers, both internal and external. Uses multiple electronic platforms including, but not limited to, Microsoft Office, Epic (EMR software), Care Port referral system, iPhone, copy/scan/fax.
What you will do
- Promote teamwork by working collaboratively as well as independently. Seek assistance as needed.
- Deliver and answer questions about Medicare notices to patients. Accurately document delivery of the form(s) in Epic.
- Accurately and efficiently upload, scan, or fax clinical information to assist with referrals to outside care facilities and agencies.
- Schedule hospital follow-up primary care provider appointments.
- Assist with scheduling meetings. Complete meeting notes and attendance records.
- Assist with monthly audits.
- Deliver durable medical equipment to the bedside.
- Ability to run reports in Epic.
- Mail certified letters to patients as needed.
- Maintain office supply stock.
Qualifications
Education
- High School Diploma or GED, (Required)
Experience
- 1 year Administrative Experience (Preferred)
- 1 year Health Care Setting Experience (Preferred)
