Who you are:
- Advanced and demonstrated skills using MS Office suite required.
- Prefer graduates of formal programs in medical assisting or experienced medical assistants, Home Health Aides or Certified Nursing Assistants. College graduates without experience also considered. Must have solid work ethic and demonstrated experience taking initiative.
- Must be neat and well-groomed and have a courteous, friendly, and pleasant manner.
- Ability to understand and follow instructions in English, communicate clearly and effectively both orally and in writing.
- Experience working with seniors strongly desired. Experience and/or knowledge of dementia a plus.
General Summary:
The Wellness Center Coordinator oversees the operations of the Wellness Center, liaises between Residents and visiting medical providers, connects Residents with community resources and provides administrative support to Residential Health Services Director and Care Navigator.
Essential Job Duties:
- Acts as primary contact for residents calling or visiting the Wellness Center. Answers telephone and schedules resident appointments with visiting providers. Explains logistics related to appointments (e.g., documents to bring,) but is not authorized to provide patient education. Requests/submits referrals. manages calendar showing when provider visits are scheduled and communicates with residents and providers offices regarding any schedule changes.
- Coordination of health services with four specialized healthcare providers: Audiologists, Dental Hygienist, Nurse Practitioner, and Podiatrist supported patient scheduling, registration, referrals, and appointment reminders, while maintaining medical and personal records as per HIPAA regulations. Updates insurance information. Manages information such as lab and imaging orders, physician-ordered physical, occupational and speech therapy referrals, and filing of records.
- Acts as gatekeeper and appointment scheduler for Director and Care Navigator.
- Using MS Office Suite, produces professional, eye-catching collateral to promote department's services. Updates and maintains internal media sources (newsletters) to promote department's services. With Director's directions and supervision, produce presentations and presentation materials to educate residents, staff and families within the community about department's services and relevant wellness related topics.
- Produces well=written copy for weekly newsletters, educational materials promoting department's services and relevant wellness related topics.
- Provides ongoing administrative support to Director and Care Navigator including clerical assistance, gatekeeping, calendar management, problem-solving, topical research, and any other assistance as needed.
- Oversees distribution, programming and tracking of call-for-help personal pendant alarms.
- Manages office supplies and medical supplies. Coordinates durable medical equipment delivery/return. Maintains stock of patient education materials.
- Monitors Wellness Center, Reception Desk and Lobby area and ensures the areas are clean and orderly and exam rooms are ready for visiting providers. Submits work orders for required repairs.
- Serves on the Health Care Advocacy Committee as the Secretary.
- Coordinates new resident meetings to acquaint them with Residential Health Services and the services the department offers.
- Organizes periodic vaccination clinics (flu, covid, shingles, etc.) to promote accessibility of vaccination resources for residents.
- Follows up with hospitals regarding residents after ER visits and hospital and skilled nursing stays to coordinate care and ensure smooth transitions home as needed.
- Assists in onboarding new visiting providers.
- Develops and updates list of alternative external health care resources (such as primary care, alternative skilled nursing, et.)
- Tracks Independent Living Security Incidents from the Security reports. Uses this information to provide relevant data to resident and management stakeholders.
- Tracks cases and create reports for COVID-19, Norovirus, and any other infections and diseases prevalent in the community.
- Other duties as assigned.
PHYSICAL REQUIREMENTS/WORKING CONDITIONS:
Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and/or HBV; must be able to transport residents via wheelchair. Must be able to stoop, bend, stretch, squat, stand, and walk for up to 75% of the workday. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 pounds frequently, and 50 pounds, occasionally.
