Job Overview
- Admit residents from hospitals in accordance with established policies and procedures.
- Check patient referral portals consistently and communicate with Hospital Case Managers promptly regarding referrals.
- Effectively communicate with Administrator, Nursing staff, Admissions Director, and Community Liaison about prospective patients.
- Obtain the resident/guardian’s signature on all required permits, releases, authorizations, etc.
- Maintain a current listing of all residents, their level of care, and assigned room numbers.
- Notify nursing service when the resident has arrived at the facility to assure that a member of the nursing staff escorts the resident to his/her assigned room.
- Maintain a resident waiting list in accordance with established policies and procedures.

Education / Requirements
- High School or Equivalent
- Experience working in a long term care setting (Skilled Nursing Facility)
