Job description

Admissions Intake Coordinator DUTIES AND RESPONSIBILITIES:

  • Must be familiar with the La Fuente Policies and Procedures, will apply and adhere to all LFHTC rules and regulations.
  • Will be up to date with and can effectively communicate the La Fuente mission/vision, the population served/why La Fuente is unique, levels of care offered as well as the cost of treatment and insurance policies accepted at La Fuente.
  • Is the first point of contact for anyone seeking treatment and or admission information at La Fuente.
  • Understands and adheres to the La Fuente Incidental Medical Services intake policies and protocols.
  • Will answer the admission phone as needed.
  • Provides phone pre-assessments as needed to ensure possible clients seeking treatment are appropriate for La Fuente. Will set up phone interviews with the clinical team when exclusions are present in the pre-assessment.
  • Gather health insurance information for verification of benefits as well as the financial resources needed for each client to ensure admission to La Fuente.
  • Is responsible for coordinating intake and ongoing client care between the various departments - clinical, medical, operations, and admissions for the duration of treatment.
  • Ensures that each client receives client assignment notebooks, group schedules and copies of rules/regulations, signed treatment agreements, and orients the client to facility and program.
  • Coordinates with LVNs, HCPs, pharmacy, and staff – to obtain new prescriptions, medication refills and to request and/or report medication changes as needed.
  • Prepares, processes and audits all admissions paperwork/client charts accurately (i.e., within CARF and State guidelines). Maintains an accurate tracking system to review documentation and ensure compliance. Closes client paper charts upon discharge per all licensing, certifying, and accreditation requirements. Will report discrepancies immediately to department heads via email.
  • Will assist clinical in determining client initial case management needs (ex. FMLA, disability, letter to employers, lab work, emergency dental, etc.). Will coordinate appointments and documentation as needed.
  • Is responsible for updating “Client Status Log” with intake information.
  • Demonstrates ability to prioritize and manage multiple responsibilities/tasks simultaneously in a timely manner without compromising quality such as accuracy, thoroughness.
  • Will coordinate with Admissions Manager and Clinical Director to ensure that all information needed on potential and new clients to La Fuente is provided. (insurance information, financial commitment)
  • Will set up a payment schedule with reminders for cash pay clients as needed.
  • Directs and assists with treatment program admissions, facility tours, consult meetings with potential clients, and all other inquiries.
  • Meets weekly with the clinical and administrative team to give updates on possible admits at La Fuente.
  • Sends a current weekly census report to the CEO, Admissions Manager, and Clinical Director.
  • Meets with the CEO and Clinical Director weekly to provide updates on admissions and to discuss overall strategy.
  • Identifies local networking opportunities that contribute to increasing and maintaining census at La Fuente. The area covered will be primarily Los Angeles County with occasional visits to SF, SD, LB, and other parts of the country if needed.
  • Works with the web-master to update any and all changes needed (services offered, staff bios, etc.)
  • Ensures that collateral and business cards are updated and stocked.
  • Creates email campaigns, maintains and continues building La Fuente database on Constant Contact.
  • Must be able to lift up to 30lbs if needed

OTHER:

  • Provides crisis intervention when needed, and immediately reports outcomes directly to the clinical team.

QUALIFICATIONS:

  • Must agree to abstain from drugs or alcohol during the time of employment. Must have been sober for at least two years.
  • Complies with LFHTC Code of Conduct, Client, and LFHTC Confidentiality Agreement.
  • Qualified applicants must be free of any non-drug-related violent felony convictions.
  • Clean driving record preferred.
  • Must have experience working with the LGBTQ community. Or Knowledge about the LGBTQ community.
  • Basic Microsoft Office, Constant Contact, and WordPress knowledge required.

Job Type: Part-time

Pay: $18.00 - $19.00 per hour
