- Develop and expand opportunities for community integration and participation to address personas served social and recreational interests
- Assist in completing the ISP process for DDS funded individuals
- Respond to emergency needs of individuals
- Oversee financial budgets, balance check books, bank deposit, timely payment of bills of persons on assigned caseload
- Attend meetings with various agencies to address the needs of individuals
- Ensure and safeguard CCI program members' dignity, rights, and liberties.
- High School Diploma or General Education Degree (GED)
- Bachelor's Degree in human service related field preferred
- 1+ years experience and/or training working with individuals with disabilities
- 1+ years experience in case management
- Valid driver's license, access to a motor vehicle, and satisfactory driving record
- Ability to work a flexible schedule
- Strong written and verbal communication skills
- Demonstrated ability to work independently
- Strong computer and word processing skills
- An advocate for individuals with disabilities
- Someone who is a team player but can succeed independently as well
