The Case Management position is responsible for providing quality care to child, adolescent, adult and geriatric patients, and their families and guardians throughout the patients' episode of care. Quality care includes, but is not limited to the following: performing psychosocial assessments, participating in multi-disciplinary rounds and treatment planning meetings, providing care coordination, organizing individual/family/systems meetings, facilitating clinical process groups, managing the discharge and continued care planning process and documenting all patient encounters.
-Review of patient's clinical documentation from referring facilities
-Conduct check ins with all clients minimally twice weekly
-Complete timely and thorough documentation:
- Psychosocial Assessment
- Outreach and care coordination with patient's providers
- Initial Multidisciplinary Treatment Plan
- Treatment Planning updates
- Progress notes
- Discharge Plan
-Participate in monthly Case Management Departmental Meetings
-Participate in process/quality improvement measures
-Facilitate process groups
-Complete required CPI, CPR and other orientation and annual hospital trainings
-Maintain appropriate state licensure
