We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Case Manager, you will be responsible for coordinating patient care and ensuring the delivery of high-quality services. Duties:
- Conduct comprehensive assessments of patients' medical conditions and needs
- Develop individualized care plans based on patients' goals and treatment options
- Collaborate with healthcare professionals to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to the care plan
- Provide education and support to patients and their families regarding their medical conditions
- Conduct diagnostic evaluations and interpret test results
- Utilize your knowledge of physiology, medical terminology, and medical equipment to provide optimal patient care
- Coordinate referrals to specialists as needed
- Advocate for patients' rights and ensure they receive appropriate resources and services
- Maintain accurate and up-to-date documentation of patient interactions and care provided
Qualifications:
- Valid RN license in MA
- Minimum of 2 years of nursing experience, preferably in case management or a related field
- Strong knowledge of case management principles, practices, and regulations
- COVID vaccine required + booster & Flu vaccine
- BLS
- Ability to work independently as well as collaboratively with a multidisciplinary team
- Proficient in medical terminology and electronic health record systems

Pay: $3,225.00 - $3,423.00 per week
Schedule:
- 8 hour shift
- Day shift
- Monday to Friday
- Night shift
- Weekends as needed
