Job Description
Monitors preparation and delivery of medication and prescription by Technicians. Provides drug information to other members of the healthcare team including nursing staff in-services. Provides pharmaceutical care to assigned patient care unit including patient education and discharge planning; drug therapy monitoring and interventions.
ESSENITAL FUNCTIONS OF THE JOB:
- Monitors the preparation and assures the quality of I.V. admixtures and patient medication orders prepared by the Pharmacy/ I.V. Technicians.
- Assures the appropriate and timely delivery of medication to the patient; assures compliance with legal requirements for distribution of controlled and non-controlled substances.
- Ensures accuracy of computerized patient-specific medication profile.
- Reviews charts and medication profiles to assess potential toxicity, drug compatibility, interaction potential, and patient response.
- Provides drug information to nurses, physicians, and other members of the healthcare team; makes recommendations regarding the use of the optimizing drug therapy; makes recommendations regarding the use of non-formulary system; interprets laboratory data related to drug therapy.
- Initiates, monitors, and maintains drug therapy protocols as assigned.
- Ensures accuracy in the medication process, including transport, storage, security, and/or administration of medication to include IV solutions.
- Participates in technician, nursing, and other ancillary services education and orientation. Researches literature prepares and presents material for lectures and formal consultations and for presentations to local community groups.
- Participates in inter- and intra- departmental quality improvement activities including narcotic audits, monitors drug utilizations; performs targeted drug studies for rational therapy; cost effectiveness, and formulary maintenance recommendations
- Represents Pharmacy Services in interdisciplinary meetings and on committees as assigned.
- Participates in the retrieval and review of reported adverse drug reactions.
- Participates in pharmacotherapy based programs mandated by regulatory bodies include protocol management, monitoring and documentation.
- Maintains pharmacy license in good standing and continuing education requirements and records as mandated by the State of Maryland, Board of Pharmacy, attends department and hospital designated mandatory in-services.

Qualifications
MINIMUM EDUCATION, EXPERIENCE, AND LICENSE/CERTIFICATION REQUIERED:
- Bachelor of Science degree in Pharmacy or a Pharmacy Doctorate degree from accredited School of Pharmacy. Prior experience as a hospital pharmacistpreferred. Current Maryland Pharmacy License. Required to rotate shifts,weekends and holidays, as scheduled.
- The employee in this position must be able to demonstrate the knowledge and skills necessary to provide care based on the age of the patients served.
- He/she must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status, to interpret the appropriate information needed to identify each patients requirements relative to his/her age specific needs, and to provide the necessary care as described in the department’s policies and procedures.
