Responsibilities:
- Develop and implement individualized family service plans for clients with developmental disabilities and their families
- Coordinate and monitor services provided by various agencies and providers
- Collaborate with clients, families, and caregivers to ensure effective service delivery
- Maintain accurate documentation and records of client enrolled in the program
- Advocate for clients' rights and access to community resources
Qualifications:
- Associates degree or higher in social work or related field
- Experience working with individuals with developmental disabilities
- Strong administrative skills
- Excellent communication and interpersonal skills
- Ability to manage multiple tasks and prioritize effectively
