CHCW has an opening for an RN Care Coordinator to join our CONNECT team. The CONNECT Program offers medication assisted treatment (MAT) for patients with opioid dependence and opioid use disorder. The Care Coordinator works with patients to plan & monitor care for medication assisted treatment (MAT), facilitate patient access to appropriate medical and specialty providers.This position is located in Ellensburg.
Job duties include:
- Assists care providers during MAT clinics and provides patient education
- Schedules patient appointments and coordinates care related to MAT services
- Manages the waiting list and assists patients waiting or entering the program
- Provides clear and concise documentation of each element of planned care goals
- Provides teaching to patients/families related to substance use disorder (SUD) treatment with Suboxone
Qualifications:
- Current WA State RN License required
- Two (2) years of related nursing experience required
- Prior experience with Substance Use Disorder is preferred
- Current driver’s license and auto insurance required
