Job Summary
The Resident Care Coordinator (RCC) is a non-professional employee trained to provide personal care and related services to residents in the community according to documented Individual Service Plan(s) ("ISP"). May be a Certified Nurse's Aide but this is not required. The RCC also assists in the training of, supervision and motivation of Resident Care Partners and other ancillary duties as described herein.
Responsibilities
- The RCC is responsible for the complete implementation of the ISP specific to each resident, ensuring that person centered care is explained in a way that fosters consistent approach and results.
- Ensures that any additional communication systems such as bath lists, laundry lists, daily task list, etc. reflect each resident's person-centered needs.
- Reads shift to shift reports daily when on duty; maintain these books as needed.
- Monitors the task completion in the Surpass system at least weekly and monitors for changes in resident needs. Monitoring of changes includes verbally reviewing resident needs at least monthly to note any changes that may not have been documented. At least monthly a comparison of the ISP and actual care delivered should be performed, if changes are noted, these changes are reported to DRC and ED in writing.
- May provide assistance with the following ADLs/lADLs according to the resident's ISP.
- Communicate with supervisors regarding residents' status.
- Utilize standard precautions and infection control procedures.
- Assist residents in maintaining a safe and clean environment.
- Update Resident Care plans.
- Maintain resident records/charts, filing and archiving.
- Ensure resident record/charts contain all state required documents, etc.
- Incident Reports
- Performs any other duties as assigned.
Leadership
- Embrace the surpass vision and core values and provide leadership that promotes these values.
- Solve day-to-day HR and care issues with the RCPs and ED direction/support as needed.
- Serve as a day-to-day coach and mentor to develop RCPs and ensure proper execution of job duties.
- Assist in delivering corrective action with the approval of the ED or DRC as needed.
- Attend monthly CQI/QAPI meetings.
Schedule: Sunday- Thursday
