PeaceHealth is seeking a Program Coordinator Advanced Care Planning for a Part Time, 0.50 FTE, Day position. The general salary range for this job opening at PeaceHealth is $33.23 – $49.85. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.
Job Summary
The Program Coordinator will be responsible for providing coordination and successful implementation of Honoring Choices Advanced Care Planning (ACP) Initiative throughout their assigned Network. Will provide facilitator mentoring and continued support to PHMG staff, hospital staff, specialty clinic staff, Hospice and Palliative Care staff. Provides direct supervision to volunteer ACP instructors and facilitators (where applicable).
Details of the position
- Under the guidance and leadership of the Director of Palliative Care and the Advance Care Planning Manager plan/develop/ implement ACP infrastructure within network including: education, workflow, documentation, measurement and reporting.
- Participates and coordinates network multidisciplinary steering committee to ensure sustainable ACP service across the care continuum in the network.
- Coordinates ACP implementation and integration within the network current care continuum model.
- Triage ACP referrals and determine their appropriateness and resource need
- Provide clinic based services for those who need one-to-one support
- Offer education and mentoring for all staff around advance Care Planning
- Performs other duties as assigned.

- Bachelor's Degree Required: Healthcare. or equivalent work experience in healthcare industry or
- Master's Degree Preferred
- Required Upon Hire: Basic Life Support and
- Required within 6 months: Certified Facilitator with Respecting Choices and
