Job description

The Specialty Nurse Navigator works throughout the Specialty Care Services and uses the Nursing Process and Evidence-Based Practice to collaborate amongst the Specialty Care Teams (Patients, Specialty Providers, Clinical, and Clinical Support Teams) and larger team (family/caregiver, internal / external customers, and community-based services involved in providing care to the veteran) in developing the patient-driven outcomes.Qualifications: Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Medicine Specialty - Outpatient Experience - Minimum 3 years.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
The Four Dimensions of Nursing are:
1. Practice
2. Professional Development
3. Collaboration
4. Scientific Inquiry
Within the Four Dimensions of Nursing, nine criteria define the performance requirements for RNs at each grade and/or level in the Nurse Qualification Standard. These requirements listed below, are based on the American Nurses Association (ANA) Standards of Care and Standards of Professional Performance.
1. Practice. Extent to which the RN effectively uses the nursing process components of assessment, diagnosis, outcome identification, planning, implementation, and evaluation in varied practice settings.
2. Quality of Care. Extent to which the RN systematically evaluates and improves the quality and effectiveness of nursing practice and health care delivery.
3. Performance. Extent to which the RN evaluates his/her own nursing practice as well as the performance of others.
4. Education/Career Development. Extent to which the RN acquires and uses current knowledge for self and others.
5. Collegiality. Extent to which the RN contributes to the professional development of peers, colleagues, and others.
6. Ethics. Extent to which the RN makes decisions and takes action in an ethical manner.
7. Collaboration. Extent to which the RN collaborates with clients, significant others, and other health care and service providers.
8. Research. Extent to which the RN uses research in practice.
9. Resource Utilization. Extent to which the RN considers factors related to safety, effectiveness, and cost in planning and delivering care.
The above criteria are conceptualized within the Four Dimensions of Nursing as follows:
1. Practice (Practice, Ethics, Resource Utilization).
2. Professional Development (Education/Career Development, Performance).
3. Collaboration (Collaboration, Collegiality)4. Scientific Inquiry (Quality of Care, Research).
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: Heavy lifting and carrying (45+ lbs.); Moderate lifting (15-44 lbs.); Light lifting (under 15 lbs.); Heavy carrying (45+ lbs.); Moderate carrying (15-44 lbs.); light carrying (under 15 lbs.); pushing (1 hours); reaching above shoulder; use of fingers; both hands required; walking (6 hours); standing (2 hours); kneeling (1 hours); ability for rapid mental and muscular coordination simultaneously; both eyes required; depth perception; ability to distinguish basic colors; hearing (aid permitted); Work may be performed outside and inside constant noise;; unusual fatigue factors: mental fatigue working with hands in water; working closely with others.Education: IMPORTANT: A transcript must be submitted with your application if you are basing all or part of your qualifications on education.
Length of Nursing Practice (Experience). The amount of time documented on VA Form 10- 2850a or on a candidate's resume. (A performance evaluation or reference covering the candidate's most recent employment as a RN is essential.) Part-time experience as a RN is credited according to the relationship it bears to the full-time workweek. For example, a RN who worked 20 hours a week (i.e., on a half-time basis) would receive one full-time workweek of credit for each 2 weeks of such service.
Length of LPN Practice (Experience). LPN experience may be counted towards successful nursing practice for an RN at the rate of ½ credit as compared to full credit for RN experience. The amount of time that may be credited toward RN experience may not exceed 5 years. For example, LPN experience would be counted as one full-time workweek of credit for each 2 weeks of such service. (A performance evaluation or reference covering the candidate's most recent employment as an LPN is essential.
Degree in a Related Field. Baccalaureate and graduate degrees in fields related to nursing from a college or university which was accredited by the state at the time the candidate completed the program. Information on accredited colleges and universities is contained in Accredited Institutions of Post-secondary Education, published annually by the American Council on Education, One Dupont Circle NW, Washington, DC 20036 or call (202) 939-9300. Information can be obtained from the ACE web site.
