Program Coordinator – Homeless to Housing (H2H) - $20.63/Hr

FT 40 Hr/wkHomeless to Housing (H2H) is an initiative designed to provide street outreach and engagement services, housing search and move-in, and linkages to community resources for people who are suffering from serious mental illness and/or substance abuse and are homeless or at imminent risk of becoming homeless in Eastern Connecticut. Supports are on-going once the individual is housed.

Duties and Responsibilities:
- Assists in implementing policies and procedures for one or more programs, and reports status to Unit Director
- Assists in data collection for required reporting and program evaluation
- Provides rehabilitation, case management, crisis intervention and supports to clients as needed
- Assists in the development of, and monitors, the effectiveness of Service Plans that are designed by a team and integrated with other program services and natural supports, and reports status to Unit Director
- Enters data into HMIS as designated by Director
- Attends team meetings as designated by Director
- Provides program admission as designated by Director
- Creates IRP, Crisis Plan and risk assessments with clinical consultation and as designated by Director
- Provides direct care according to program standards, percentage of time will vary.
- Maintains pertinent and current documentation in member(s) file(s)
- Assists Unit Director in assessments of admissions, discharges, and staff assignments for Program participants
- Assists in training new staff in their roles
- Provides conditions that maximize safety for staff and program participants
- Provides coverage as needed at any of the Agency sites per Agency need
- Works Split shifts including weekends primarily, according to program standards
- When applicable, assists in and maintains an employee schedule that meets the needs of the program participants, the program, and staff
- Provides AIC duties as scheduled
- Engages in continuous concerted efforts to access, obtain, respond to pertinent Agency information/communications and disseminate as appropriate
- Other responsibilities of Reliance Health staff such as participation in Agency events, adhering to Agency Policies, training requirements etc.
- Other job-related duties as assigned
- Performs duties while demonstrating Agency Core Competencies and values
Education and Experience:
- Preference given to those with lived experience
- Bachelor’s Degree in Human Services or equivalent work experience in the provision of mental health preferred
- High School Diploma or equivalent required
- Valid CT Driver’s License, vehicle registration and insurance
- Good driving record
