Job Description
- Coordinate and oversee non-Medical Home Care services for clients.
- Conduct client assessments to determine care needs and develop individualized care plans.
- Match clients with appropriate caregivers based on needs and preferences.
- Monitor client progress and adjust care plans as necessary.
- Communicate effectively with clients, caregivers, and other healthcare professionals.
- Ensure compliance with regulatory standards and company policies.
- Provide support and guidance to clients and their families.
Requirements:
1. Bachelor's degree in Health Care Management or related field (preferred).
2. Previous experience in Home Care coordination or case management.
3. Strong communication and interpersonal skills.
4. Ability to multitask and prioritize tasks effectively.
5. Compassionate and empathetic attitude towards clients and their families.
6. Knowledge of local community resources and support services.
7. Flexibility to adapt to changing client needs and schedules."
Job Types: Part-time, Temporary
Pay: $19.63 - $23.64 per hour
Expected hours: 20 – 30 per week
