JOB TITLE: Home-Based Care Coordinator
DEPARTMENT: Senior Services Department
PROGRAM: Supportive Services Program STATUS: Full Time -Non-Exempt
REPORTS TO: Senior Services Manager HOURS PER WEEK: 40
POSITION SUMMARY:
· Under the supervision of the Senior Services Manager, supervises staff that provides personal care and homemaking services for seniors and respite for their caregivers.
DUTIES AND RESPONSIBILITIES:
- Supervise all aspects of the Home-Based Care programs, including Registry, Home Care services (personal care and homemaking)
- Plan, organize and direct staff duties and schedules on a weekly basis
- Monthly trainings of new registry applicants
- Intake/Screen clients via telephone and/or face-to-face interview for eligibility of services, including making appropriate referrals as deemed necessary
- Maintain accurate client files with up-to-date activities recorded on progress notes
- Maintain daily logbook of activities, i.e., information and assistance, referrals, etc.
- Maintain accurate program statistics, services rendered, and receipt of payment for services on a monthly basis
- Compile billing logs and enter data into GetCare
- Participate in supervisory meetings, and attend other meetings, as necessary
- Related duties, as required
KNOWLEDGE, SKILLS AND ABILITIES:
- Proven competent skills, as well as the ability to exercise independent and good judgment.
- Knowledge of elder and dependent adult abuse warning signs and risk factors.
- Knowledge of community resources and other social service agencies.
- Ability to relate well with caregivers/family members of clients.
- Good verbal, written communication, and organizing skills, especially regarding listening and understanding client needs.
- Ability to follow policies and procedures.
- Ability to meet attendance standards.
- Maintain high level of confidentiality.
- Develop and maintain effective working relationships.
- Computer literate to include Microsoft Office software.
- Preferred Bilingual (English/Spanish) highly desirable.
EDUCATION AND EXPERIENCE:
· Bachelor’s degree in gerontology, social work, or related field is preferred or a high school diploma, or equivalent is acceptable with experience
· Prior training in gerontology, dementia, Alzheimer’s, care management, or related training; one (1) year of experience working with senior citizens which includes also communicating with their family members and/or caregivers.
LICENSING/CERTIFICATION REQUIREMENTS:
· Driving is required. Must provide own vehicle, valid California driver license and proof of full insurance coverage.
· Negative tuberculosis screen (required).
· Must pass criminal background check.

The Care Manager’s work location is in Bell Gardens and Monrovia, CA offices with constant travel throughout Los Angeles County; the majority of the job duties are performed out in the community visiting one to multiple clients in any given day to check on their safety and welfare and to perform assessments in the home; the employee may need to sit or stand for short or long periods accordingly to home environment and/or due to assessment as well as stair and hill climbing accordingly. The employee will complete paperwork in the office as required; the location has moderate noise levels and is a non-smoking environment; employee must climb up one flight of stairs, and occasionally lift and/or move up to 25 pounds.

Pay: $24.00 - $28.00 per hour
