The Manager of Case Management is responsible for the daily operations of the Case Management team. The Manager will coordinate with Physicians, Nurses, Social Workers and other health team members to make sure processes are in place to expedite medically appropriate, cost-effective care. The Manager assists the Director with the development of guidelines for the Case Managers to apply their clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. The Manager will coordinate the daily activities of the Case Management team and provide leadership as needed.
Education:
- Associate or Bachelor's degree in nursing required
Certifications/Experience:
- Minimum of 3-5 years’ experience in acute care Case Management experience required
- RN Licensure required
- Bachelor's degree in a health related field and CCM certification is preferred.
