Job Description
To conduct comprehensive assessments of all referrals and individuals requesting in Home Services. To advocate on behalf of all individuals for needed services and assistance and make appropriate referrals. To coordinate, monitor and follow-up on Home and Community Based Services for individuals as directed.
Main Responsibilities
• To conduct in-depth assessments of individuals challenges, resources, functioning, needs and to develop a written plan of care in conjunction with the individual, the significant people in their life and Program eligibility that is person centered
• To conduct assessments in the presence of the individual and whenever possible, in their preferred setting and to encourage participation of family and friends as approved by the individual.
• To provide information to referrals and their representatives concerning all potential service delivery methods including private pay, formal supports, informal supports or government funded services. To provide individuals and/or families with accurate and complete information regarding the extent and nature of the services available to them
• To maintain professional standards of client confidentiality. To discuss cases only for professional purposes and only with persons directly and professionally concerned with them
• To be in compliance with HIPPA, Federal and State Guidelines & Comprehensive Case Management Standards
• To coordinate service delivery with community resource agencies to ensure that necessary services as determined through assessment and documented in the care plan and eligibility screen will be delivered to the individual
• To monitor/evaluate service delivery at least quarterly to determine if quality and quantity of service meets individual’s needs
• To reassess the individual’s needs for services and be in compliance with requirements of all programs and complete all required documentation of the visit
• To make every effort to foster maximum self-determination on the part of individuals and to respect their self autonomy
• To provide individuals and/or families with accurate and complete information regarding the extent and nature of the series available to them whether from SWIRCA or another community resource
• To complete daily, monthly and quarterly reports and documentation accurately and timely

Qualifications
Baccalaureate Degree in Social Work, Psychology, Gerontology, Health & Human Services or Registered Nurse licensed to practice in the state of Indiana and an Indiana Case Management Certification preferred. Any Baccalaureate Degree acceptable. Experience assisting the aged and disabled populations preferred. Must be proficient in Microsoft Word, Excel and email Familiarity with a variety of the field’s concepts, practices, and procedures helpful. Current Driver’s License and use of Automobile required.

While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts. The environment is usually quiet to moderate.
