Job Summary: Provide Skills Training, in group and individual settings, and case management services to adult clients of the Family Health Center.
Essential Job Duties:
1. Conduct assessments of assigned clients within timeline identified by policy.
2. Develop and complete Person Centered Treatment Plans (PCTP) and treatment plan reviews with clients within timeline identified by policy.
3. Provide services to clients and on behalf of clients as outlined in the PCTP.
4. Enter progress notes, complete State Registration and ANSA documents, maintain appointment book in EMR, and complete billing documentation within timeline identified by policy.
5. Participate in team meetings, consult and cooperate with team members regarding shared caseload tasks, and contribute actively to the team’s efforts to ensure clients’ needs are met.
6. Follow designated curriculum when conducting skills training, maintain adequate supplies and materials needed for skills training, and complete skills training tasks according to the designated schedule/timeline.
7. Maintain knowledge of Medicaid Rehabilitation Option (MRO) guidelines regarding provision of services, documentation requirements, qualifying diagnoses, and level of need issues.
Job Specifications:
a. Education: Bachelor’s degree in a field related to mental health is required.
b. Preferable Experience: Two (2) years' experience in community resource acquisition and rehabilitation services for adults with serious mental illness
