Job Summary:
Performs general intake procedures for the Client Services Department. Duties may include assisting with applications for public assistance, Social Security, and housing programs applications. Research and networks with community resources for referral purposes as well as other administrative assignments. Develop and maintain a community resource guide. Communicates with service providers regarding client needs and follow-up with the client’s Case Manager. Conduct follow-up on all referrals and documents referral outcomes.
Responsibilities Include:
- Demonstrates sensitivity to the cultural needs of a diverse client population.
- Administrative tasks i.e., data entry, intakes, phone calls, follow-up and referrals.
- Work with Case Managers on special client projects. Inform Case Manager of immediate needs.
- Provides advocacy, support, and linkages/referrals to community resources.
Qualifications Include:
- Minimum degree requirement - associate degree in human services or comparable
- Experience working in a multi-cultural environment for a not-for-profit organization providing direct service to the economically disadvantaged
- One or more years’ experience in Case Management
- One or more years’ experience in Group Facilitation
- Knowledge of Community Resources
