Description
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Intake Specialist is responsible for processing referrals received from RiseBoro marketing and partner organizations in a caring and professional manner. He/She/They must understand the regulations governing the homecare field, related to Medicare, Medicaid and other insurances. The Intake Specialist coordinates with multiple parties including the patient, family members, external organizations and Homecare staff to optimize the intake experience.
Key responsibilities are listed below:
- Liaise with clients, family members, referral sources (HRA, CASA, MCO and MLTC offices), CHHA's, hospitals nursing homes, and all other entities to facilitate client admission and service coordination
- Work with Homecare team to make preliminary determination on assigned client referrals and ensure timely and accurate transmission of information to responsible parties
- Provide guidance and assistance to new or potential home care patients throughout the onboarding process
- Communicate directly with new or existing patients to provide education and guidance
- Ensure responsive and prompt coordination of client enrollment, disenrollment, enrollment issues and service hour modifications
- Maintain an up-to-date and accurate intake, outreach and case referral log
- Continuously consult, coordinate with and provide follow-up information for marketing and business development staff
- Coordinate all outreach efforts with Lead Intake Specialist and marketing team
- Work with the Homecare marketing team to optimize the client relationship management system in place and optimize client acquisition systems and processes
- Advocate for client needs with respective HRA, CASA, MCO and MLTC offices and other service providers
- Conduct extensive review and quality assurance of all assigned required Homecare intake documentation
- Consult and follows-up with the appropriate case management agency regarding changes in clients' status
- Gather encode and transmit all documents and information relevant to case deferrals and rejections for analysis
- Encode and/or update new or re-admitted client information into the HC Plus system
- Work in coordination with responsible Homecare units and designated HRA, CASA, MCO and MLTC offices to process client deferrals/rejections, authorization documentation, start/end dates, re-admission dates, and billing requirements
- Work with responsible Homecare staff to maintain the accuracy of authorization, status change, discontinuation, M11A, M11V, and any other required forms
- Assist the Lead Outreach Specialist and leadership in the areas of customer service, quality assurance, intake and outreach analysis, and the generation of client outcome reports
- Prepare routine case notes and reports as required
- Maintain a current resource file on entitlements and services available for our service population
- Ensure that all hard-copy and electronic client information is secure at all times in accordance with Homecare and Riseboro security protocols
- Participate in team/skill-building and professional development activities
- Work closely with Lead Intake Specialist and leadership to promote and maintain a supportive, positive and welcoming working environment, which reflects the mission, vision and values of RiseBoro
COMPETENCIES
- Bi-lingual English/Spanish (highly preferred)
- Exemplary interpersonal and customer service skills
- Solid working knowledge of Homecare specific database management and Microsoft Office products
- Able to effectively multi-task, work under pressure, implement systems, follow up processes, and use independent judgement to produce quality work within tight deadlines
- Highly organized with superb attention to detail; able to gather and analyze information skillfully
- Demonstrated ability to work with a diverse population, contribute to building a positive team spirit
