Intake Coordinator
Role:
Setting up SupportiveCare Medical Group new patients across all care settings
Responsibilities:
- Focal point of all new patients
- Available for weekend on-call rotation
- Verifies insurance eligibility to make sure the provider visit is billable
- Retrieves all vital documents for in-hospital visits (e.g. facesheet, progress note, and H&P) and all vital information for non-hospital referral patients
- Adds the necessary information to internal “tracking sheet” and EMR system
- Collaborates with community partners to schedule hospital patient consultations
- Communicates on a regular basis using SupportiveCare standards (e.g. SBAR)
- Manages data from multiple internal and external sources (e.g. EMR’s, faxes, emails, etc.)
- Drives communications in group texts between SupportiveCare Medical Group and community partners to make sure updates between the parties are within a timely manner and are to follow specific protocols for special appointments (e.g. compassionate extubations)
- Update status across calendar and shared spreadsheets until patient is discharges or signs onto GIP services
- Proactively identify issues and works with the leadership team to resolve problems and improve community partner relationships, quality, and productivity
Qualifications:
- High school diploma
- 18 years or older
- Basic computer skills and telephone etiquette
- Solutions-driven problem solver

Job Type: Full-time
Pay: $17.50 - $20.00 per hour
Expected hours: 40 per week
