Intake Coordinator for Home Health Job Description:
Managing patient intake process, establishing, and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
Essential Job Functions/Responsibilities
1. Evaluates hospitalized patients referred for home care.
2. Works under the supervision of a registered nurse.
3. Identifies and verifies insurance coverage of home health care services.
4. Obtains and documents prior authorization for home care services from insurance providers.
5. Negotiates per diem or per visit payment rates for creative bundling of home care services.
6. Collaborates with the hospital's discharge planning personnel, utilization review department and insurance Case Managers to facilitate safe, early discharge.
7. Completes referral information that includes intake data, essential background information, hospital course, and the plan of care.
8. Maintains liaison relationship with hospital and insurance personnel, providing information and education on Organization services, coverage issues and related areas.
9. Ensures compliance with all state, federal, and CHAP referral/Intake regulatory requirements.
10. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
11. Establishes and maintains positive working relationships with current and potential referral sources.
12. Ensures seamless transition of patients to home care.
13. Consults with physicians, nurses, social workers, discharge planers and other disciplines to prepare for the creation of the plan of care.
14. Builds and monitors community and customer perceptions of ALC Home Health Care, inc. as a high-quality provider of services.
15. Gathers, collates, and reports referral statistics including key customer referral trends and submits them regularly as required.
16. Attends meetings and participates on committees as required.
17. Performs other duties as assigned.
Position Qualifications
1. High school diploma required.
3. Minimum of two (2) years' experience, at least one (1) of which is in public health or home care nursing.
4. Demonstrates excellent observation verbal and written communication skills.
